hmm let's see...

the basic set:
-water
-plaster
-wallet
-phone
-keys
-pump
-tyre levers
-spear tube
-patch kit
-Pedros multitool that also has chain tool and a bottle opener
-pins for the chain
-swiss knife
-mini torch
-shock pump (this is stupid I have never used it when been out)

That I carry always.
I also carry a compact camera often.
Depending on the weather maybe a hoodie or something to put on if gets colder.
food depending on the day. ranges from a banana or home made energystuff (like energybars) to a whole set for a good picnic. a bottle of beer is luxory.
This all fits in my Osprey Talon 11 that I'm very happy to own.
For shorter trips I can leave it home though and carry nothing and that is a nice feeling!

edit:
Interested on hearing other peoples must-have-in-bags I might have forgotten something important! A spear mech hanger would be good to have.

Liquid
Trail Snacks
Pump
Shock Pump
2x Inner Tubes
Puncture Repair Kit
Allen Keys
Chain Tool
Spare Mech Hanger
Small First Aid Kit
Waterproof (Depending on the weather)
Extra Top, usually a long sleeve cotton T-Shirt (Again weather dependant)
Wallet/Keys
Sometimes take a Compact Camera

MOST IMPORTANTLY in case it all goes wrong I take the Mobile Phone!

All crammed in a Dakine Drafter pack, sometimes I am surprised I manage to get the thing closed!

If I am going out locally I don't always bother taking a pack out with me.

Much the same as Affe. I take some zip-ties and beer money too. Keep your phone safe - I had a crash once and landed on my phone.

I don't carry anywhere near as much as I used, but currently for a normal ride-
Water
1 Inner Tube
One extra layer of clothing
Tyre Levers
Multi-tool kit with couple spare power links
Big set of Allen Keys
17mm Spanner (for the ghetto 12mm rear wheel axle)
Minipump
Spare Mech Hangars (one for each bike)
Couple apples/bananas
Mobile phone

Oh yeah, I also have spare cleats and screws.

We were out riding one day in the Perth hills, a long way out, and a mate of mine put the rear mech of his Nomad into the spokes and did all sorts of damage. We looked at it grimly and contemplated a long walk home. A few minutes late, some cat turns up who was both a bike mechanic and who was carrying a pretty good set of tools. Soon after, we were riding again. For an epic ride, bring a bike mechanic!

Mobile phone!

The heavy and sometimes usefull stuff I let the others carry for me, such as: power bars, Red Bull cans, chocolate cookies, pizza, beer (allways usefull), first aid kit, allen keys, spare chains, spare derailleurs, spare tires, spare bikes...

Oh, almost forgot... one heavy and usefull thing I always carry: A BIG PAIR OF 'COHONES'!  Grin Grin Grin

Cheers,

Nuno

Tubes
Tool
Compressed air canisters (instead of pump)
Arms for my gillet (my fav item of clothing)
Old mobile phone (pre pay with some credit..just in case)
Bannana
Keys
Other stuff depending on the ride, but this is the basics

I have two packs, a big kona for day rides/commuting. And a smaller pack for local rides

I leave my "BIG PAIR OF 'COHONES'" at home in case they get damaged Lips Sealed

I only gotz a little camelback, so i carry with me;

Water, about 2.5L,
Allen key set, from 1.5mm up to 10mm.
park tyre levers
park self adhesive patches
some spare tyre levers
High pressure Shock Pump (surprisingly useful)
Tyre pump, with schraeder and presta fitments, i've had to help many people out on the trail before as they didn't prepare.
MOBILE PHONE
Wallet (With personal ID, and personal details if the worst happens)
House/car keys.
Varying amounts of money.

And what stays in the car;

Change of clothes/SOCKS/trainers
Towel
Chain lube/GT85
Spare pair of gloves.

liquid
munchies
set of allen keys
multitool with 'everything' (27 tools on the bugger)
small set of plier/grip things (last use:tweaking a rotor back)
chain tool
puncture kit
spare tube
2 toothpaste tubes slit lengthways (for sidewall splits)
chain pins
zip ties
2 spare gear cables
pump
shock pump
another foldaway set of allen keys
1 or 2 rags (for cleaning mud away in the event of a mechanical)
craft knife
lighter
pen Huh?
wallet
loose cash
keys (with some extra chain links on the keyring - just in case)
phone
map

That all comes with me whether I'm out for an hour or the day. The only time I've come a cropper is when my cassete lockring came loose but I'm not carrying that tool around with me!

Sometimes take a camera and a short extendable fishing bankstick with an adapter to mount the camera for self takes.
Keep meaning to add a small first aid kit and a random selection of spare bolts.
